The webinar will demonstrate how Hampshire County Council’s Adults’ Health and Care put the Live Longer Better Public Health Strategy into practice, showcasing innovative models such as the Family Plan. It will share practical interventions that help older adults maintain independence and feature real-life case studies with measurable impact. Designed for social workers and practitioners, the session offers actionable insights to improve wellbeing and reduce health risks.
